SIG Topic:  “Prolonged Exposure: An Evidence-Based Treatment for PTSD”

Prolonged Exposure (PE) is an evidenced-based, manualized therapy effective for the treatment of Posttraumatic Stress Disorder (PTSD).PE has demonstrated efficacy in diagnostically complex populations and survivors of single- and multiple-incident traumas and is endorsed by the American Psychiatric Association, the Department of Veterans Affairs, and Substance Abuse and Mental Health Services Administration (SAMHSA).PE is based on Emotional Processing Theory, which posits that PTSD symptoms arise as a result of avoidance of trauma-related thoughts, reminders, activities, and situations, as well as trauma-related thoughts and beliefs. While avoidance serves a protective function, individuals often experience decreased quality of life and perpetuated symptoms of PTSD. The four primary components of PE include 1) psychoeducation about treatment and common reactions to trauma, 2) breathing retraining, 3) in-vivo exposures, and 4) imaginal exposures. Exposures are used to gradually and systematically confront distressing memories and situations to facilitate emotional processing and organization of the trauma memory, discriminate safe from unsafe situations, reduce excessive fear, and increase mastery and confidence. Exposures also serve to block avoidance and allow opportunities for new and corrective learning. In addition to treatment overview and clinical implications, special issues will be discussed, including PTSD versus trauma, avoidance, under-engagement, and over-engagement.
